 Listed below are a list of all the operative consent forms for the Women's Plastic Surgery Centre.
Once your surgery has been scheduled, you will receive a pre-operative packet of information in the mail. Enclosed in the packet will be a "Surgery Summary" Form, which will list the procedures that you will be having. Please refer to this list and download the appropriate consents from this page.
