On Your First Visit
Please bring your insurance card and any required insurance forms. We will ask you to complete our new patient forms.
Payment Arrangements
We will be happy to submit your insurance claim forms for you. We also accept Visa, Master Card, AMEX, Discover, check or cash.
Financing Options
For those patients interested in financing their procedures, we offer the following financing options listed below. Patients with excellent credit obtain extremely competitive interest rates.
Prescription Refills
Please call our office Monday through Thursday, 8:00 a.m. until 5:00 p.m or Friday, 8:00 a.m. until 12:00 p.m.
Lab Services
We will make all necessary arrangements for your lab tests or help you coordinate with your primary care physician.
In An Emergency
Call our office at (513) 891-5610. We maintain a 24 hour answering service. During normal office hours (8 a.m. - 5 p.m. Monday through Thursday, Friday 8 a.m. - 12 p.m.), our staff will answer your calls and connect you with Dr. Butterfield. After office hours, the doctor will return your calls.
